W-2 INFORMATION
For employees of itsQuest, Quest Healthcare, JPSS and JR Staffing

W-2s will be out by January 30th, 2008. If you do not receive your W-2, you may request a copy by mail or our online request form.

1. Postal Mail Request - Send a written request and a SASE (Self Addressed Stamped Envelope) OR a 41 cent postage stamp to:

The written request must include your full name, Social Security number, name of the branch(es) you worked out of, and your current mailing address.

2. Online W-2 Request (referenced below) - You should receive your W-2 within 72 hours (during normal business week days only) if you choose the Online W-2 Request. You will receive the file in Adobe Acrobat Reader format - click here to download a free copy. (*We can only provide W-2's for wages earned while working as employees of itsQuest, Quest Healthcare, JPSS and JR Staffing.If you worked for any other company, you need to contact that company directly for your W-2)

*Online W-2 requests will be filled only AFTER the original W-2 have been mailed.

W-2s from previous years (2002 and earlier) are available by e-mail, it may take an extra couple of days to locate the form, and scan it for you.
